Please notify our office in writing of any changes of address, telephone numbers, billing or contact information. All medical record requests must be on a HIPPA approved form, which must be completed and signed by a parent or legal guardian. Please allow at least five business days for processing any forms. Medical records released to a new provider, specialist, or school will be forwarded at no charge. Records released to the patient, some insurance companies, law firms, or other requests may be subject to copying fees.
We accept cash, Visa, MasterCard, and Discover as payment options. All payments for services are due on the day services are provided, unless prior arrangements have been made with our billing office. Any remaining balances or past-due amounts will be collected at check-in.
If you are late for a sick appointment, we will try our best to work your child in at the next available appointment time. If you are late for a well-child appointment, we will make every effort to work you in, if the schedule allows. In certain cases, we may need to reschedule for another day. Please give the office 24 hours’ notice of any cancellations, so we may offer that time to another patient. Excessive no-shows may be subject to a fee.
